When: Monday, May 13th from 4:00-6:00p
Details: Please complete, and bring the completed Medical History form and copy of blank physical (attached) with you for each physical needed.
The cost for the sports physical is $10 and will be billed to your account.
PLEASE KEEP COMPLETED PHYSICALS TO UPLOAD TO RANK ONE. Instructions will be sent over the summer through Team App on when to begin uploading forms to your child’s profile on rankone.com.
